Kindergarten and Students New to Wichita Public Schools
Parents of students entering kindergarten and students new to the Wichita Public Schools should bring the following items with them to enroll:
• Birth Certificate – If a birth certificate is not available, schools will accept a hospital certificate of birth, a visa or passport, a certified transcript from another school district or a court order that shows the birth date and places the student with Social and Rehabilitation Services. Students may enter kindergarten if they will be five years old on or before Aug. 31, 2009. Students may enroll in first grade if they will be six years old on or before Aug. 31, 2009.
• Social Security Number (optional) – All students are assigned a six-digit ID number by the computer. A student's Social Security Number (SSN) may be added to the student record as a separate field of data. This information is useful to the student in tracking his or her records or for providing other government services. A SSN may be requested, but it cannot be required.
• Kansas Health Assessment – Students up to age nine who have never attended a Kansas school should present the results of a health assessment upon entering school. Students will not be excluded from attending school if they do not have the assessment, but they are strongly encouraged to complete this form. To comply with this law, 1992 KSA 72-5214, the assessment:
- Must be conducted by a physician, a person working under the direct supervision of a physician and cosigned by that physician or a nurse who has special certification from the state to perform these assessments. Click here for the form. Only one form per child is needed.
- May be done up to 12 months prior to school entry or up to 90 days after school entry.
- May be recorded on any health assessment form that includes health history, physical exam and screening tests as medically indicated by the provider. Wichita Public Schools’ Health Examination Report, a yellow card, may be used.
- Applies only to students age eight and under who have never been enrolled in any Kansas school.
- Results must be given to the school by the parent or guardian. These records will be kept in the student’s folder and transferred to a new school if they change schools.
• Immunization Requirements – Before your child may enter school anywhere in Kansas, you are required by state law (KAR 72-5208, 5209, 5210, 5211, 5211a) to provide the school with documentation of your child’s immunizations. This immunization must be provided to the Wichita Public Schools on an official Kansas Certificate of Immunization KCI Form, available from your neighborhood school or most health care providers, and and at the Sedgwick County Health Department, Clinical Services 2716 W.
