31-Day Rule for Benefits Enrollment:
Employees who experience a Life Event (marriage, divorce, birth, or adoption) have 31 days from the event date to add new dependents to the health plan or delete dependents.
If new dependents (new babies or new spouses) are not added within the 31-day period the employee must wait until Annual Open Enrollment to add them to the Health plan.
Health Insurance
The Wichita Public Schools group health insurance plan is a self-funded plan including health, dental, vision and pharmacy coverage for all eligible employees, retirees and their qualified dependents. The district hires third party administrators to pay claims and manage their respective networks.

Preapproval is required for all mental health care and substance abuse visits.

Out Of Area Health Plan Dependents
Coventry has a "Kansas Passport Program" for registering your out-of-area dependent so they will have in-network health services. Dependents who reside in an area that is not in the Coventry network, including students who to to an out-of-state school, should be enrolled in the "Kansas Passport Program" so their health care will be handled as "in-network" care. This document has instructions on registering your dependent. If you have questions call Coventry at the USD 259 toll-free number listed on your Coventry card.

Full-time Student Verification requirements
If your dependent is between ages 19-23, and is a full-time student, they can stay on your health insurance with proof of their current full-time student status. Coventry will send a letter requesting verification of student status to all affected health plan members. Verification of student status is required to keep the dependent on the health plan.

MHNET - Mental Health and Substance Abuse Provider
Our Mental Health Network is with MHNet. All inpatient and outpatient mental health and substance abuse services will require prior authorization from MHNet before services are rendered. Call MHNet at 1-866-607-5970.

MEDCO - Mail-order Pharmacy Provider
Beginning January 1, 2010, MEDCO will be our new mail-order pharmacy provider.
A list of covered medications can be viewed online at www.chckansas.com.
Click on Plan Members.
Click on Prescription Formulary to get a copy of the formulary list.
Choose the 2010 Alphabetical Formulary List, 2010 Tiered Formulary List, or click on Enter Online Formulary Search. The Prescription Drug list provides a comparison of Generic, Preferred, and Non-Preferred medications.

Some drugs are not available through mail order. Please check the Coventry website for which drugs can be ordered in a 93-day supply. (Examples: Migraine headache drugs, antibiotics, anti-fungals, anti-emetics, controlled substances, and some self-administered injectables.)
To Submit or Refill and Order
To place an initial order by mail, you must:
(1) Complete a Medco Order Form. You must supply your Coventry medical ID information from your medical card.
(2) Obtain an original handwritten prescription from the prescribing doctor.
(3) Determine your payment method (check or credit card).
(4) Mail the order form, prescription and payment to Medco.
Refills can be ordered by phone 24 hours a day by calling the Customer Service toll-free number on the front of your Coventry card and selecting Pharmacy; by mail, using a Medco order form; or online at www.chckansas.com and click on Prescription Benefits. If your prescription has refills remaining, your Medco prescription label and receipt both alert you to when you should refill your order. Your prescription number, member ID number, and credit card information will be needed when ordering refills.
New orders may take up to two weeks to receive – refills typically within a few days. Overnight or two-day delivery is available at an additional cost.
Primary Care Physicians (PCP): This generally includes physicians who practice in the specialties of Family Practice, Internal Medicine, General Practice, or Pediatrics. If you are not sure if a physician is a PCP, please contact Coventry Customer Service using the USD 259 employee toll-free number on the back of your health card.

LIFE INSURANCE -- For Benefited District Employees
The district has group term life insurance through The Standard that covers all eligible benefited employees and retirees. All employees with district life insurance should review/verify your beneficiaries online through the eBenefits program. Changes to beneficiaries, or changes to the allocation of life insurance among beneficiaries, can be made through eBenefits any time during the year.
Updates to your KPERS Life Insurance beneficiaries can also be made at any time. These beneficiary changes must be made by submitting a new KPERS Beneficiary Form to the KPERS offices in Topeka. This form must be filled out completely, signed and witnessed, and then mailed or faxed to KPERS for the changes to be posted. The mailing address and FAX number are on the beneficiary form.
SUPPLEMENTAL LIFE INSURANCE - Voluntary Benefit
All benefited employees may elect to enroll in the Supplemental Life Insurance program within the first 31 days of employment. Employees must go online through the eBenefits enrollment system to enroll. The company who carries the district Supplemental Life Insurance program is The Guardian.
