Primary Care Physicians (PCP): This generally includes physicians who practice in the specialties of Family Practice, Internal Medicine, General Practice, or Pediatrics. If you are not sure if a physician is a PCP, please contact Coventry Customer Service at the toll-free number on the back of your health card.

Life Insurance:
The district has group term life insurance that covers all eligible employees and retirees. Employee groups are covered at these levels:
Certified Teachers: $30,000
Classified A-Ji: $30,000
Classified K-Z: $40,000
Administrators: $50,000
All eligible employees should review your district Life Insurance beneficiaries online through the eBenefits program. Changes to beneficiaries, or changes to the allocation of life insurance among beneficiaries, can be made through eBenefits at any time.
Updates to your KPERS Life Insurance beneficiaries must be made by submitting a new form to KPERS. Find their form at this website:
KPERS Beneficiary Form. This form must be completed, signed and witnessed, and mailed or faxed to KPERS for the changes to be posted.
