BENEFITS OPEN ENROLLMENT PERIOD IS NOVEMBER 2-12, 2009
Open Enrollment for the new plan year (January through December, 2010) will be held from November 2nd through November 12th, 2009. All Employees are required to go online and enroll for benefits, including the Health plan and the Health premiums, and the FLEX Spending plans. The Health Plan at a Glance for 2010 and Health Plan Highlights documents will help you make your decisions on your Health plan for the new year. We also have an Enrollment Quick Guide to help step you through the online enrollment process. TOWN HALL MEETINGSThe slides from the Benefits Town Hall meetings are available for everyone to view. Benefits Town Hall Meeting slides HEALTH PLAN 2010 INFORMATIONThe Medical plan changes for 2010 are noted in the Health Plan at a Glance for 2010 document . The premiums, deductibles and co-pays are listed for 2009 and 2010 plan years. On page two you will find a reminder of the Free Annual Preventive Care programs (through in-network providers).
January 1st, 2010, employees will begin paying an Annual Health Premium of $240. Employees may waive this premium by participating in 4 Wellness Activities . Open Enrollment for employees will take place during November, 2009. At this time you will choose which Health plan you will participate in for the 2010 plan year. The Pharmacy enhancements for 2010 will include $0 co-pay for generic blood pressure and cholesterol medications. The current medications covered by this change are listed here.
HEALTH PLAN 2009 District Employees will be on a short health plan from September through December 2009. We will then move the benefits to run on a calendar year, from January through December of each year. The short year will have prorated premiums, deductibles and co-insurance (for in-network services). The changes for this 4-month plan year are outlined in the Summary Plan Document.

COVENTRY HEALTH ASSESSMENT TOOL
Coventry has a Health Assessment tool that is available for all members of the USD 259 Health Plan. This tool will allow you to store and track your personal health information. The instructions to log in to the Health Assessment are attached. Coventry Health Assessment Instructions
* * * * * * * * * * * * * * * 31-Day Rule for Benefits Enrollment
Employees who experience a Life Event (marriage, divorce, birth, or adoption) have 31 days from the event date to add new dependents to the health plan. If new dependents are not added within the 31-day period the employee must wait until Annual Open Enrollment to add them. The 31-day period also applies to removing dependents due to a divorce.
LIFE EVENT: The date of the birth or adoption of a child, or the date of a marriage or divorce.
